PelEX was invited to participate in the first 504HealthNet Connected Care Summit, hosted at the University Medical Center in New Orleans. The summit gathered 504HealthNet members to discuss the current and future state of care coordination and ways to improve it between member organizations.

The summit featured a panel of impactful leaders who continue to advocate for safety net organizations and a series of member showcases where 504HealthNet organizations shared their work.

Many PelEX participants are 504HealthNet members, and our team was excited to further engage with our participants alongside new partners and connections. Our team co-designed an interactive groupthink session for participants to answer essential questions about our care coordination landscape.

The major takeaways from this activity revealed priority needs for 504HealthNet and PelEX to address for our shared members, including:

  • Improve data reporting. There are limits to evaluating patient success and health outcomes beyond the volume of service provided
  • Center patient experience. Organizations can avoid duplicating patient intake processes by developing a shared care model.
  • Integrate data systems. Instead of “hub and spoke” networks where users must access different databases, we can build an interconnected “web” of data systems that can eventually integrate referral and medical data for better care coordination.
  • Build trust with providers. Our organizations can do a better job of providing education on cybersecurity and privacy governance measures.


Now officially partnered, PelEX and 504HealthNet hope to address these priorities through continued engagement, making connections between members, and investigating data resources and leveraging existing tools to understand where gaps can to be filled.
