Member Showcases

People are the core element in driving processes and using technology. PelEX’s mission is to provide actionable products to people who are engaged and willing to incorporate them into their workflows to find ways to align efforts and improve efficiency. PelEX is focused on engaging people to better understand how our products can meet immediate needs and directly support their process development.

At our member event, participants will be hearing from the people in process at two PelEX member organizations. 

Bridging the Gap: Leveraging Hospital Utilization Reports to Improve Continuity of Care
Metropolitan Human Services District

This presentation explores how weekly Hospital Utilization Reports can be used as a powerful tool to enhance care coordination within a mental health agency. By tracking emergency room visits and hospital admissions, agencies can promptly alert providers and prescribers when their patients experience acute care episodes. This proactive approach fosters timely outreach, keeps patients engaged with their primary agency contacts, and facilitates critical referrals to external physicians, specialists, and support services. The result is a more comprehensive, responsive care system that addresses both mental and physical health needs—improving outcomes and reducing hospital recidivism.

Dr. Desiree Stelly, DNP, RN

Dr. Desiree Stelly, DNP, RN is a dedicated leader and lifelong learner in the fields of nursing and healthcare innovation. She currently serves as the Division Director of Nursing at Metropolitan Human Services District (MHSD), where she brings a wealth of clinical and academic expertise to her role. Dr. Stelly previously spent four years as a Simulation Educator at Ross University School of Medicine and one year as a Lecturer in the Master of Nursing program at the University of the West Indies, Cave Hill Campus. She holds active licensure in 32 U.S. states and two countries. A proud member of Sigma Theta Tau International Honor Society of Nursing, Dr. Stelly is also the founder of Nurses Care, an NCLEX support program designed to empower and equip aspiring nurses for licensure success.

Dr. Dominique London, Pharm.D.

Dr. Dominique London, Pharm.D. is a dedicated healthcare professional with a background in medical writing and healthcare system operations. She currently serves as the Pharmacy Liaison and Crisis Fidelity Monitor at Metropolitan Human Services District, where she bridges clinical insight with operational strategy to improve service delivery. With a passion for making subtle yet powerful impacts, Dr. London is committed to transforming lives and advancing the health of the communities she serves through thoughtful, data-informed care and system-level innovation.

How People, Processes, and Technologies Hindered a Universal Screening Effort to Address Patients' Non-Medical Needs + Finding Solutions to Move Forward
Access Health Louisiana

This session will describe the technical, operational, and resource barriers one FQHC encountered trying to successfully implement a universal screening initiative to identify and report on patients’ non-medical needs in accordance with state-level Medicaid Managed Care Organizations’ requirements.   Strategies to overcome these challenges and preliminary findings will be shared.   

Participants will resonate with common barriers and challenges, and leave with ideas to improve workflows.   The presenters also hope that this session will highlight the complexities associated with universal screening and reporting for patients’ non-medical needs across multiple Medicaid MCOs and influence solutions to streamline and simplify the process for engaged community partners.

Chatrian Roberson, MPH

Chatrian Roberson, MPH is the Senior Vice President of Population Health with Access Health Louisiana Community Health Center, where she is responsible for aligning resources and clinical information systems to leverage performance improvement initiatives across our network. Chatrian works across partners, health plans, departments and care teams to devise care coordination systems aimed at improving quality, reducing costs, and monitoring patient outcomes.

Lee Reilly, RN

Lee Reilly, RN is currently the Director of Value-Based Contracts and Network Performance with Access Health Louisiana Community Health Center. She has been a nurse for 48 years, has maintained a Case Management Certification for over 30 years, and recently obtained her FQHC coding and billing certification. In her current leadership role, she provides oversight and implementation of value-based health initiatives while targeting specific cost savings areas through evidence-based protocols and innovative care management strategies. Prior to joining the Access Health team, Lee worked in the managed care industry for over 30 years and in physician practices for 10 years.
